Our hospital's Geriatric Emergency  Team has developed an "Integrated Case Management System" using information technology to enhance healthcare services for the elderly. This system provides real-time medical information and resources for discharged patients, and allows physicians to track post-discharge progress through case managers, minimizing potential medical disputes.
