The hospital has developed an integrated patient-centered information system: the Actue Kidney Injury Electronic Alert System and the Acute Kidney Disease Tracking System. The systems apply algorithms and historical database for automatically matching and define the occurrence and severity of acute kidney injury. The "Chronic Kidney Disease Map System" provides a dashboard showing the location of patients and the status of kidney disease. It facilitates the intervention process by professional multidisciplinary teams. The Chronic Kidney Disease Case Management System(CKD DMS) automatically integrates information for easier patient-centered management and tracking. Through the intelligent systems including data, knowledge, practice and patient, the experience of CKD CMS is a successful model of artficial intelligent health-care system.
