The purpose of this study is to design a continuous care record exchange platform to address the difficulty of integrating health and welfare care. By using electronic health records and Fast Healthcare Interoperability Resources (FHIR), the platform will consolidate records from health promotion clinics, medical care facilities, and long-term care facilities to construct an integrated medical record.
