The intensive care unit (ICU) is one of the most demanding care environments in a hospital, where the density of care is highest and the need for timely decision-making is most critical. Healthcare professionals must monitor critically ill patients around the clock while also facing multiple challenges, including interdisciplinary team coordination, integration of complex medical data, and heavy workforce demands.

To address these core issues, China Medical University Hospital brought together expertise in medicine, information technology, and engineering to develop the HiThings Tele-ICU Intelligent Critical Care Command Center.

Built on a high-performance IoT platform, the system integrates medical devices, electronic medical records, medical imaging, and AI applications in real time. Through both the Unit Overview Dashboard and the Patient Digital Twin Dashboard, it helps care teams quickly understand the overall status of the ICU as well as changes in individual patients.

The platform also provides real-time AI alerts, clinical decision support, and teleconsultation capabilities, with access across multiple devices including computers, tablets, and smartphones. Together, these features create a real-time, intelligent, and comprehensive critical care model that significantly improves care efficiency, response speed, patient safety, and overall quality of care.

Benefits

Since its initial launch in the Respiratory ICU at the end of 2023, HiThings Tele-ICU has been expanded across intensive care units within our hospital and affiliated healthcare system, covering cardiac, surgical, medical, and pediatric critical care. Its clinical effectiveness has now been well validated.

In terms of quality of care, the ARDiTex AI supports physicians in the early identification of acute respiratory distress syndrome (ARDS), significantly improving diagnostic rates and reducing mortality. The infection control map provides real-time visualization of the distribution of drug-resistant organisms, strengthening hospital infection prevention and control. The NutriPilot system delivers personalized nutritional recommendations for critically ill patients, increasing the rate of achieving calorie and protein targets. Overall, mortality in the Respiratory ICU has shown a sustained decline in recent years, demonstrating significant clinical impact.

In terms of patient safety, the PADIS map enables real-time monitoring of pain, agitation, delirium, and related conditions, helping healthcare staff intervene earlier and reduce adverse events.

In reducing the burden on healthcare staff, the Patient Digital Twin Dashboard integrates fragmented clinical information and substantially shortens information retrieval time. MIRA, a generative AI solution combined with speech recognition, automatically produces structured medical documentation and reduces charting workload. The equipment tracking system also improves handover efficiency and optimizes nursing workflows.

From a cost-effectiveness perspective, the system is less expensive to deploy than comparable international solutions, while offering high scalability and availability, making it a highly competitive smart healthcare solution for hospitals.

In terms of dissemination and international impact, the system is planned for expansion to multiple affiliated hospitals. It has also been showcased at the HIMSS Global Health Conference in the United States, HIMSS APAC in South Korea, and MEDICA in Germany, and has received international recognition including the AI Impact Awards APAC and the Healthcare Asia Awards, demonstrating the global competitiveness of Taiwan’s smart healthcare innovation.
